How to Start with our Home Care Services
At Home Care Solutions, care begins with an initial consultation with a care coordinator to determine the specific needs of the client. If the client is in the hospital, the initial consultation is most beneficial before discharge.
Our care consultants meet with discharge planners – MD’s and the therapists – so that care is most effectively carried over to the home. In most cases, potential caregivers will also be present during the initial visit.
